Fuel Control Switch Malfunction

  • Just after take-off, both fuel
    control switches moved from
    "run" to "cut-off," leading
    to a loss of engine thrust.
  • Conversation between the pilots:
    one asked why the cut-off occurred,
    and the other denied any action.     ...

So there you are, a suicide-minded
pilot is still in a position to kill all
aboard simply by flicking the fuel
control switches – at least on a
Boeing with GE engines.  And he
shouldn't be – certainly not by
flicking those switches, as the
Engine Management System should
be perfectly capable of rejecting
the fuel cut-off command in an
airliner at a critically low altitude,
even if the engines are on fire –
and even if the cut-off command
is sent from a physical switch
(unless such switch is executed
in two parts, spaced wide apart,
to be operated by both pilots).
Anyone purporting to be a
programmer should have been
capable of getting this piece of
crucial basic logic right, but not
Boeing or GE, as it appears.

the fuel
US officials believe the captain of
the Air India jet that crashed shortly
after take-off may have been the one
who cut off the aircraft's fuel supply.

According to a US assessment of
black box data, the first officer, who
was piloting the Boeing Dreamliner,
questioned why the captain had moved
switches to the cut-off position during
the critical moments of take-off.

The Wall Street Journal, which reviewed
the findings, reported that the first
officer expressed “surprise and panic”
while the captain remained calm. ...

So it came to that hapless co-pilot's
having to ask the bastard this rather
belated question – instead of the
Flight Control System immediately,
loudly and punitively screaming 
at him and telling him what to do – 
and, of course, never executing his
suicidal command, in the first place.

There is more than enough redundant
intelligence both within the Boeing
Flight Control System and the GE
Engine Management System to
obstruct suicidal commands and to
alert the rest of the crew instantly
to the presence of a saboteur
in the cockpit.
